Services

Balance, strength, & optimal health

Therapists at Thrive In-Home Rehab examine all factors that impact joint-specific motion and functional mobility. The results allow us to create an evidence-based treatment plan and establish patient-centered goals to optimize health.

Many older adults struggle with chronic disease which can lead to progressive, functional decline and dependence. Thrive In-Home Rehab provides education and interventions to prevent or rehabilitate impairments related to these disease processes to improve quality of life and safety.

Our team specializes in the care of geriatric clients to maximize independence and application of health, safety, and exercise programs.

Fall risk reduction & prevention

According to the National Safety Council, falls are the leading cause of accidental death in adults over the age of 65. Injuries sustained from falls are costly and devastating to patients and their families. Additionally, fear of falling can lead to activity limitation and progressive weakness which increases actual fall risk.

Therapists at Thrive In-Home Rehab provide a comprehensive assessment of fall risk factors and household barriers. An individualized treatment plan is created to address personal risk factors such as balance, strength, and flexibility. Collaboration with the medical team supports resolution of other health risks such as vision and medication management. Thrive In-Home Rehab maintains awareness of community resources to aide home safety modifications and provides specific training to lessen the impact of environmental barriers.

In-Home Physical Therapy

Thrive In-Home Rehab exists to provide one-on-one therapy treatments where patients live to address the physical and environmental limitations that lead to pain, falls, and dependence.

After thorough examination, an individualized exercise program is created that can be conveniently performed and progressed at home. Though goal-oriented interventions are paramount, we strive to make exercise a lifelong daily routine that is continued by each patient even after therapy is completed.
